Where a bridge-out failed AFTER GalaConnect accepted its RequestTokenBridgeOut (BRIDGE_OUT_OUTCOME_UNKNOWN.params.step):

• request_id: the answer had no bridge request id;
• bridge_token_out: the BridgeTokenOut call failed;
• transaction_hash: the BridgeTokenOut answer had no transaction hash.
